These are substantive terms with editable particulars and schedules. Complete the fields and obtain the required approval before issuing an individual agreement. This public template is not a signed contract. No personal information entered below is sent to NurseFlex or stored by this page.
Entries update this page only. Word and PDF downloads remain blank templates. Check the preview and every schedule before issue. Do not enter patient records, bank details or identity documents here.
This schedule applies only to the sites, role categories, dates and bookings expressly covered. Pay and charge rates are distinct. The total charge and any separate tax must be clear before confirmation. No blank rate, premium, expense or cancellation field is treated as an agreed additional charge. A positive lawful worker rate and accepted positive provider charge are required before publication or supply.
2 Changes
A change applies only from its agreed effective date and must be accepted by authorised representatives. It does not retrospectively change confirmed bookings without agreement or reduce statutory worker rights. The parties retain the accepted version and the calculation supporting the invoice.
3 Cancellation and credits
A cancellation amount is recoverable only where its calculation and triggering notice were agreed before the booking. Record the notice timestamp, affected hours, replacement or mitigation and any worker compensation. Do not charge twice for the same item. A no-show is not time worked. Any service credit must identify the booking and reason and does not authorise unlawful withholding of worker pay.
Acceptance and signatures
The parties accept the completed particulars, clauses and schedules by their recorded signature or agreed electronic acceptance. This is a simple contract and is not executed as a deed.

For new offers, advertised inclusive staff totals contain basic pay and a 12.07% holiday component only for evidenced eligible irregular-hours or part-year PAYE arrangements. Basic pay and holiday pay are itemised separately. The holiday component must not be added twice. Regular-hours and other accrued-holiday arrangements are paid for leave under their applicable terms; an illustrative inclusive guide is not all current shift cash. Staff gross figures are before employee tax, National Insurance and pension deductions. Weekly payday is Friday, subject to the agreed timesheet approval cut-off. Existing signed terms and accepted assignments are unchanged.
Weekday day basic rates: nurse £21.4152, HCA £13.50 and Support Worker £14.00. Payroll uses exact basic rates and rounds earnings and holiday for the pay period; rounded hourly components can differ by pennies.

Standard client figures are proposals until the facility, role, category, effective date and agreement are confirmed. They include the recorded holiday and employer NI/pension provisions plus the service fee. Employer-cost provisions are commercial estimates, not individual payroll deductions. VAT remains subject to the recorded supply assessment. Employer NI is a NurseFlex cost and must not be described as part of staff take-home pay. Accepted assignments retain their locked basic, holiday and client prices.
Bank holiday takes priority, then weekend, then weekday night or day. Weekday night is 20:00–07:00 Europe/London. Cross-boundary hours require the applicable agreed rates; premiums do not stack.
For an eligible nurse, 12 paid weekday hours produce £256.98 basic + £31.02 holiday = £288 gross before employee deductions. Unpaid breaks reduce paid hours.
